Applying for MYLINE
- Click on "Get an application form". You can request an application form from our website.
- Select "Application for MYLINE and KDDI phone service (Discount service)" and enter your personal information.
We will send you the application form. 
- Fill in the "MYLINE Application" and place seals in the appropriate blanks. Send back the application form using the enclosed return envelope.
- MYLINE Center will contact you later to inform you of the service start date.

- Select "Application for MYLINE and KDDI phone service (Discount service)" and enter your personal information.
We will send you the application form. - Fill in the "KDDI Discount Service Application" and place seals in the appropriate blanks. Send back the application form using the enclosed return envelope.
- If you are also applying for MYLINE or MYLINE PLUS, also fill in the "MYLINE Application".
- If you apply for MYLINE, the MYLINE Center will contact you later to inform you of the service start date.
You will be informed of the starting date of the discount service(s) by KDDI.
